How to Make a Claim for National Health Insurance?
You can select either of the options - Cashless or Reimbursement.
For Cashless Claims,
- You need to get admitted at a network hospital and furnish your cashless card and KYC details for pre-authorization by the insurer or its authorized TPA
- You have to duly fill the cashless claim application form with details from the Network Hospitals and TPA and send it to the insurer for authorization
- Your insurer will go through the relevant documents along with the medical information and cashless claim application form. Post verification, the pre-authorization letter will be issued to the hospital
- Once your treatment is complete, you only need to pay for non-medical and inadmissible expenses and sign the discharge card
- Note: If you are unable to provide the necessary medical bills and documents, the insurer might reject your cashless claim application.
For Reimbursement Claims,
- Visit any non-network hospital for undertaking your medical treatment. Once you are discharged, submit the relevant medical documents and bills along with a reimbursement claim form within 30 days.

What are the Documents Required for Making a Claim?
- Claim Form A/B/C/D as applicable duly filled and signed by insured
- Copy of Insured Person's Passport, if available (All pages)
- Copy of Insured Member Photo ID / Photo ID Proof, if Passport is not available.
- Medical practitioner's prescription advising admission.
- Hospital bills and all payment receipts (Original) for all consolidated amounts, the detailed breakup of the billed amount is required from the hospital.
- Advance payment made, if any, should be supported by a receipt.
- Discharge Summary / Discharge card (Original, Photocopy for pre/post-hospitalization claim) including a complete medical history of the patient along with other details.
- Investigation reports (original) including Insured Person's test reports from Authorized diagnostic centre for COVID -19.
- OT notes or Surgeon's certificate giving details of the operation performed, wherever applicable.
- Sticker/Invoice of the Implants, wherever applicable.
- For medicines purchased from outside, the original bill should be accompanied by prescriptions from the doctor.
- Canceled Cheque (with pre-printed name) /Copy of passbook of the proposer for electronic fund transfer type.
- KYC (Identity proof with Address) of the proposer, where claim liability is above Rs 1 Lakh as per AML Guidelines
- Legal heir/succession certificate, wherever applicable.
- Any other relevant document required by Company/TPA for assessment of the claim
Documents Required for Home Care Treatment,
- A certificate from a medical practitioner/doctor/physician advising treatment at home or consent from the insured person on availing home care benefit.
- Discharge Certificate from medical practitioner/doctor/hospital/physician that specifies the date of start and completion of home care treatment.
- Monitoring and reporting of daily health charts including records of treatment administered duly signed by the treating doctor/physician/medical practitioner
- In case home care medical treatment is taken in a non-network hospital, a prior approval letter from the insured copy to be attached) has to be sent to the insurer.
Note: The following documents are applicable for Hospitalization Expenses, AYUSH Hospitalization, and Pre-Post Hospitalization Expenses. The notification of the claim must be sent to the insurer 24 hours from the date of emergency hospitalization or cashless home care treatment. In the case of planned hospitalization, the insured must be informed at least 24 hours prior to admission to the hospital.
